The Weight Of Her Story™ Foundation, Inc. (“Foundation,” “we,” “us,” “our”) uses written consent forms to protect the privacy, dignity, and legal rights of participants, families, volunteers, and community members. This page provides a clear explanation of how our Media Release & Consent Form functions and when it is required.
The Media Release & Consent Form grants the Foundation permission to capture and use photographs, video recordings, audio recordings, and written or spoken testimonials of participants for nonprofit purposes, including:
Signing the Media Release & Consent Form does not create an employment relationship and does not entitle a participant to financial compensation. Media and testimonials are contributed voluntarily to support the Foundation’s mission and public benefit work.
Consent to be photographed, filmed, or recorded is voluntary. Choosing not to sign the Media Release & Consent Form does not affect eligibility for services or participation in Foundation programs, unless a specific activity is clearly identified as requiring media participation (such as a recorded interview project).
Families may submit one form on behalf of the household, as long as all adults and minors included are clearly listed. Each adult retains the right to withdraw their own consent in the future, subject to the limitations described below.
For any minor under the age of 18, a parent or legal guardian must sign the form. The Foundation does not knowingly use identifiable media of minors without verified guardian consent.
Participation in media is optional. Individuals who do not wish to appear in photographs, video, or other identifiable media are not required to complete a form. They may:

Withdrawal of consent does not require the Foundation to remove or alter materials already created, published, or distributed before the request, unless required by law.
Participants who choose to authorize media use will be provided with the Foundation’s Media Release & Consent Form directly by staff or via official digital channels. Completed forms may be submitted by:
Media Release & Consent Forms help the Foundation verify permission for media use. Forms may be stored electronically and/or in secure physical files. Access is limited to authorized staff, contractors, or volunteers.
